Consent for Assessment and Advisement
In order to ensure the best possible care for you, we want to make sure you are aware of the purpose and function of the soul Care Ministry.
1. The Soul Care Advisor is a trained servant minister at Central Christian Church and will assess, assist, and coach you primarily through listening, prayer, and examining appropriate Scripture. The Soul Care Advisor is not a professional therapist or counselor, but someone who is equipped to provide, through God's sufficient resources, the opportunity for growth and healing.
2. Your time with your Soul Care Advisor will be approximately 50-60 minutes per session. Normally you will meet with your advisor for six to eight sessions.
3. The information you share with your advisor is generally confidential, except that the advisor is required to report to the appropriate authorities when there is reason to believe that neglect or abuse of a minor has occurred. Also, if there has been a determination that there is a clear and present threat of harm to another individual, or yourself your advisor will immediately contact the appropriate authorities. Lastly, in order to serve you better, cases are routinely discussed among the Soul Care Team without revealing personal names.
I have read and understand the above purposes and procedures of the Soul Care Ministry, and I understand that I am entering the Soul Care Ministry voluntarily and under my own free will.
